What are they doing in them?

I saw on the news yesterday that 24,000 kids per year are injured in shopping carts.

24,000? What in hell are the kids doing?

I mean, put the kid in the little seat part, put the potato chips and canned tuna in the other, and push the cart to the checkout.

It seems pretty simple to me.

Shopping carts have to be redesigned because we need to cater to the lowest common denominator, I guess.

Clearly you never participated in the prestigious under 7 shopping cart wars of the 70's, of which a resurgence has been occurring. Imagine Mad Max crossed with Quentin Tarantino and spit back at you in a gory tasting shower of bloody mayhem and you still won't come close.
